He did produce his birth certificate. It was a certified copy from the state of Hawaii (i.e., exactly what one would use in all the situations you mention). A picture of it is still available on his campaign web site.

I don't understand why this is such a burning issue with some people. Who would he have presented it to, you personally? Who? This is such a non-issue and I'll explain why. Since his mother was a US citizen, he could have been born in outer Siberia and he would be a US citizen since his mother was. Do you not think sometime in the past it was produced such as when he applied for a passport to go visit his father in Africa? Do you expect that government clerk to come forth and declare he or she saw the birth certificate?
Taking this a bit further, why haven't you asked to see the birth certificate of Joe Biden, Hillary Clinton, both George Bushes, etc., etc., etc. even Rush Limbaugh?
